HCBD administers the self-funded State of Montana Benefit Plan (State Plan). The State Plan provides healthcare benefits that support the health and well-being of state employees, legislators, retirees, and their families. HCBD partners with third-party administrators and vendors to deliver medical, dental, vision, prescription drug, and other benefits.

EOI is also known as "evidence of good health." It is the process that determines if a person is healthy enough to be considered eligible for the amount of insurance coverage they are requesting. Depending upon when you enroll for coverage, or the amount of coverage you are requesting, you may be required to complete EOI prior to coverage becoming effective.
EOI requests are submitted to BCBSMT using one of the following methods:
- Complete the form online using BCBSMT's secure EOI application.

Print and fill out the EOI form then submit to BlueCross BlueShield of Montana, Attn: Medical Underwriting Department, PO Box 7072, Downers Grove, IL 60515.

Short, easy-to-understand presentations to help State Plan members better understand their benefits and make informed decisions about their health. Topics include wellness, mental health, preventive care, Flexible Spending Account (FSA) optimization, life insurance, and navigating benefits.
The Montana Health Centers, operated by Premise Health, provide State Plan members with no-cost primary and behavioral health care, health coaching, and other wellness services. There are five locations across Montana, with virtual primary care available 24/7.
If you were eligible for State Plan benefits during the 2025 calendar year, you may request a paper copy of your IRS Form 1095-C.
Active employees and retirees may login to the benefits enrollment system to download a digital copy of their IRS Form 1095-C. Terminated employees and retirees must request a paper copy.
