State Plan Life and Accidental Death & Dismemberment (AD&D) Insurance plans are fully insured and administered by BlueCross BlueShield of Montana (BCBSMT). Plans are term life, provide inexpensive protection, do not earn cash value, and are paid after tax.

Evidence of Insurability (EOI)
EOI is also known as "evidence of good health." It is the process that determines if a person is healthy enough to be considered eligible for the amount of insurance coverage they are requesting. Depending upon when you enroll for coverage, or the amount of coverage you are requesting, you may be required to complete EOI prior to coverage becoming effective.
How to Submit EOI
EOI requests are submitted to BCBSMT using one of the following methods:
- Complete the form online using BCBSMT's secure EOI application.
OR
- Print and fill out the EOI form then submit to BlueCross BlueShield of Montana, Attn: Medical Underwriting Department, PO Box 7072, Downers Grove, IL 60515.
- The insurance group number is F026969.
Beneficiary Designation
Your enrollment in State Plan Life Insurance and/or AD&D Insurance coverage requires you to designate a beneficiary. It is important you have designated beneficiaries in the event of your death. You can add, delete, or change your beneficiary information at any time.
In the event you are divorced, please be aware of the following: Unless set forth by the express terms of a governing instrument, court order, or contract relating to the division of the marital estate between the divorced individuals, a life insurance beneficiary designation to an ex-spouse will be revoked. It is important that you update your beneficiaries upon divorce.
If a beneficiary is a minor (under age 18) or cannot give consent for payment of benefits made, a Trust must be established for the designed beneficiary.
If a beneficiary is not designated or no designated beneficiary survives you, the below will occur if payment of benefits is made:

to your spouse, if living; if not,
-
in equal shares to your living children (over age 18), if any; if none,
-
in equal shares to your parents, if living; if not,
-
in equal shares to your siblings, if living; if not,
-
to your estate.
Beneficiary resource services are available for grief and financial counseling, funeral planning, legal support, and online will preparation.
Separation of Service
Employees are eligible until separation from service. At separation, contact BCBSMT for portability or conversion options.
At retirement, Basic Life Insurance may be continued without portability or conversion until Medicare eligible if enrolling in the State Plan as a retiree. Basic Life insurance may be converted once a retiree becomes Medicare eligible.
Medicare eligible retirees on the State plan are not eligible for any group life insurance.
Portability and Conversion Information
When you lose eligibility for group life insurance coverage you are eligible to port or convert your life insurance coverage to an individual policy by making application to BCBSMT. The deadline to apply and pay premium for portability is 31 days after employment terminates. For conversion, the deadline to apply and pay premium is 31 days after coverage was reduced or ended. Note the termination date for employment may differ from the termination date for coverage. As you plan for retirement, contact BCBSMT to discuss the portability and conversion options available to you for your current life insurance coverage.
Portability and conversion are not available for AD&D coverage.
Portability: Allows you to “port” (or buy) group life insurance coverage when you lose coverage because your employment is being voluntarily or involuntarily terminated.
Conversion: Allows you to convert some or all your group life insurance coverage to an individual whole life insurance policy when your coverage is reduced or terminated for any reason other than non-payment of premiums.
Employee Only AD&D Insurance

Available during 31-day initial enrollment period.

Coverage may be elected at any time.

May elect $25,000 - $1 million of coverage in increments of $25,000.

During the annual Open Enrollment Period, employees may add, increase, decrease, or remove AD&D Insurance.
Employee and Dependent AD&D Insurance

Available during 31-day initial enrollment period.
-
Coverage may be elected at any time.
-
May elect $25,000 - $1 million of coverage in increments of $25,000.
-
A spouse with no children is eligible for 50% of the employee coverage.
-
A spouse with children is eligible for 40% of the employee coverage.
-
Children are eligible for 10% of the employee coverage.
-
During the annual Open Enrollment Period, employees may add, increase, decrease, or remove AD&D Insurance.
-
Part of the medical benefit for all active employees, legislators, and non-Medicare retirees.
-
Provides $14,000 of term life coverage.
-
No employee contribution required, paid for via the employer contribution.
-
Non-Medicare retirees pay $0.63 per month for coverage.

Support for Basic Life Insurance beneficiaries is available to provide an advance payment of the employer-paid basic life insurance benefit to help beneficiaries cover their immediate expenses, such as funeral costs and medical bills.

Available during initial 31-day enrollment period or within the first 60 days of marrying or 91 days of having your first child. Choose from Option A or Option B.

Dependent Life Option A:

$2,000 of coverage for a spouse/domestic partner

$1,000 of coverage per dependent child

Dependent Life Option B:

$4,000 of coverage for a spouse/domestic partner

$2,000 of coverage per dependent child

During the annual Open Enrollment Period, employee may remove coverage or if employee is currently enrolled in Option A, they may increase coverage to Option B.
Employee Supplemental Life Insurance I

Available during 31-day initial enrollment period without EOI.

Enrollment after 31-day initial enrollment period requires EOI.

Coverage may be elected at any time with EOI.

Coverage amount is the employee's annual salary rounded up to the next highest $5,000.
Employee Supplemental Life Insurance II
-
Coverage may be elected at any time with EOI.

Must be enrolled in Employee Supplemental Life Insurance I to qualify for Employee Supplemental Life Insurance II.

Coverage available for any amount over employee's annual salary rounded up to the next highest $5,000 with a maximum coverage amount of $1 million combined total of Employee Supplemental Life Insurance I and II.

During the annual Open Enrollment Period, employee may add, increase, or decrease Employee Supplemental Life Insurance coverage in increments of $5,000. An increase of more than $10,000 requires EOI.

Employee must be enrolled in Supplemental Employee Life Insurance.

Available during initial 31-day enrollment period up to $10,000 without EOI.

Coverage may be elected or increased at any time with EOI.

Coverage minimum is $5,000. Additional amounts are available in $5,000 increments, up to the amount of Employee Supplemental Life Insurance with a maximum coverage amount of $500,000.

Spouse’s rate is based on the employee’s age, not the spouse’s age.

During the annual Open Enrollment Period, employee may add, increase, decrease, or remove Spouse Supplemental Life Insurance.

Under the "Basic Info" section, click "Change of Beneficiary"

Enter the date you are making the change, then click "Continue"

Update your beneficiary information and follow the prompts

Primary and contingent beneficiary percentages must total 100%

Your transaction is complete and a confirmation number will be displayed
